How to create a fillable PDF form

Turn any PDF into a form people can fill in on screen, in Adobe Reader, a browser, or on a phone.

Steps

  1. Add your PDF

    Drag your PDF onto the box, or click Choose a file. It opens right in your browser. Nothing is uploaded.

  2. Pick a field type

    Choose Text field, Check box, Radio button, Dropdown, or Signature above the page.

  3. Drag a box on the page

    Drag where the answer goes, or click to drop a field of the usual size. Drag a field to move it and drag its corner to resize it.

  4. Name the field

    Give it a clear name, like Full name or Date of birth, and turn on Required if it must be answered. For dropdowns, type the choices one per line.

  5. Click Save PDF form

    The fields are written as a standard PDF form that any reader can fill in.

  6. Download the result

    Click Download to save the fillable PDF form. The original file is left exactly as it was.

The options, explained

Several lines
Lets a text field hold an address or a longer answer that wraps inside the box.
Radio button groups
Buttons with the same group name work together, so only one can be picked. Give each button its own value, like Yes and No.

Tips

  • Make each text field a little taller than the line it sits on, so the answer is easy to read.
  • Try the form in Fill PDF form before you send it, to check the fields are where you want them.

Questions

Will the form work in Adobe Reader?

Yes. It's a standard PDF form (AcroForm), the same kind Adobe Acrobat makes, so Adobe Reader and other PDF readers can fill it in.

Can I add fields to a PDF that already has some?

Yes. The fields already in the PDF are kept and shown with a dashed outline. Give your new fields names the form doesn't use yet.
